Hour Little Playhouse requires a child registration packet to accompany each child upon your child's first visit.

A $50 registration fee (non refundable) and deposit (first week's tuition) is due at time of enrollment to secure your child's space into our program. 

 

Please complete and submit the registration information along with any additional supporting documents as required by state licensing. All fees and forms are to be submitted one week before enrollment. Failure to submit one or all of the required documents and fees may result in your child not being enrolled into our program.

Enrollment Documents Required 

  • Registration Fee and Deposit

  • Child Record Form

  • Physical Examination Record  & Immunization Records (MUST be up to date and signed by physician)

  • Present Proof of Child Age & Identity  (Certified copy of child’s birth certificate)

  • Court Issued Custodial Agreements (If applicable)

Please note: The registration forms, health and immunization records, and the child’s emergency contacts MUST be updated annually and parents are required to communicate immediately if any of their information has changed.
